Altered Medical Records

If you suspect any of your medical charts have been changed or altered in any way you may need a forensic document examiner to review all medical charts, even those from presumably reputable physicians. Never assume anything. Physicians or other health care providers may have written entries or made changes in your records in order to deflect liability for medical negligence. Medical records are legal documents. Care should be taken to preserve their authenticity.
